Claims

To properly file a CLAIM with your Limited Warranty please follow the following instructions within 15 days of your repair or replacement date, and within 12 months or 50,000 miles of your application purchase date, whichever occurs first:

  1. Complete all fields on the form below
  2. Be sure to upload images or pictures of the following:
    • Windscreen Repair or Replacement Invoice
    • Payment Receipt for Repair or Replacement
    • A picture of the damaged vehicle windscreen
    • A picture of the Door Jamb Sticker with the Vehicle Data Sticker next to it (See sample image below)
